Form for official email account creation/ Wi-Fi Login/Internet connection for residence of University employee

  1. Type of Request: Dropdown list(New email Account/ New Wi-Fi login/ new internet connection)
  2. Employee Type: Radio button (Teaching/ Non-Teaching)
    If Teaching:
    Faculty: (Dropdown list data from database)
    Department: (Dropdown list data from database, filtered as per faculty)
    If Non-Teaching:
    Department: (Dropdown list data from database)
  3. Title: (Mr./ Mrs./Dr./Prof./Miss.) + Name: dropdown(both fields data from database as per filtration done above)
  4. Date: (Server’s Current date)
  5. Description of Request: (text area)

(NOTE: Please submit the printed form to the computer centre after forwarding from your department head)

  • Submit the form by authenticating through the OTP at the registered mobile no. of employee already in database.
  • And complete submission computer centre will be notified to email cc[dot]lbs[at]slbsrsv[dot]ac[dot]in.
  • After that user print the form and submit it to computer centre after forwarding from department head in PDF format.
