If Teaching:
Faculty: (Dropdown list data from database)
Department: (Dropdown list data from database, filtered as per faculty)
If Non-Teaching:
Department: (Dropdown list data from database) - Title: (Mr./ Mrs./Dr./Prof./Miss.) + Name: dropdown(both fields data from database as per filtration done above)

(NOTE: Please submit the printed form to the computer centre after forwarding from your department head)
- Submit the form by authenticating through the OTP at the registered mobile no. of employee already in database.
- And complete submission computer centre will be notified to email cc[dot]lbs[at]slbsrsv[dot]ac[dot]in.
- After that user print the form and submit it to computer centre after forwarding from department head in PDF format.
